악관절 내장증의 증상 중 연관통의 악관절 수술 후 변화

결론
저자 등은 1992년 1월부터 1997년 5월까지 악관절 장애로 연세대학교 영동세브란스병원
악관절 클리닉에 내원하여 임상검사 및 자기공명 영상 검사를 받은 환자 중 악관절 내장증
으로 진단되어 악관절원판 성형술 및 관절융기 절제술을 시행 받고 12개월 이상 정기적인
추적조사가 된 118명을 대상으로 하여 수술후 두통, 목 그리고 어깨의 통증과 악관절 내장
증의 주요 증상인 개구장애, 개구 및 폐구시의 악관절통증, 악관절 잡음의 변화를 조사한 결
과 다음과 같은 결론을 얻었다.
1. 조사대상 118명중 술전에 두통을 호소하였던 환자는 79명(66.9%)이었으며, 목과 어깨의
통증을 호소했던 환자는 각각 34명(28.8%)과 33명(28.0%)이었다.
2. 술전에 severe(+ + +)한 두통을 호소했던 32명의 환자 모두 술후 1개월 내에 두통 증
상의 현저한 호전이 있었고 26명(81.3%)은 두통이 완전히 없어졌으나, 술후 12개월에 이 중
3명(9.4%)에서 다시 severe한 두통 증상이 재발하였다.
3. 술전에 목과 어깨의 통증을 보인 환자들은 술후 1개월에 각각 91.2%와 90.9%가 증상
이 호전되었으며 술후 12개월에는 목과 어깨의 통증이 완전히 해소되었다.
4. 개구 및 폐구시 악관절 통증은 술전에 각각 79관절(66.9%)과 81관절(68.6%)에서 통증
이 있었으나 술후 12개월에는 이들 중 79.7%와 76.5%가 악관절 통증이 사라졌으며 통증이
사라지거나 호전된 정도는 연관통보다 낮았다.
5. 개구량은 술전에 평균 35.7㎜에서 술후 3개월에는 40.3㎜, 술후 12개월에는 44.9㎜로써
현저한 개선을 보였다. 관절 잡음은 술전에 58관절(49.2%)에서 관찰되었고, 술후 1개월에는
10관절로 줄어들었으나, 술후 12개월에는 28관절로 증가하였다. 따라서 관절 잡음은 술후에
하악운동범위가 늘어나면서 그 빈도가 증가하는 것으로 보이나 술후 악관절 자체의 통증 및
연관통의 호전과는 큰 상관이 없는 것 같다.
Purpose : The aim of this study was to inverstigate the improvement of referred
symptoms of TMJ internal derangement after open surgery of TMJ.
Materials and Methods : The present study focused referred symptoms such as
headache, neck pain, shoulder pain and evaluated the incidences and changes in their
improvement at 1, 3, 6, 12 months after surgery in 118 patients who had TMJ internal
derangement and received mentiscoplasty(or meiscorrhaphy) and eminectomy at TMJ
clinic of Yonsei University, Yongdong Severance Hospital, between 1992 and 1997.
Results : One hundred eighteen patients(102 females, 16 males) with mean age of 29.6
years(range, 14 to 74) were included in this study and the mean follow-up period was
19.5 months(range, 12 to 60).
The incidences of headache, neck pain, shoulder pain among the surgically treated
TMJ internal derangement patients were 66.9%, 28.8%, 28.0% before surgery. Thirty-two
preoperative severe(+++) headache patients were all improved their headache at 1, 3, 6
months after surgery, but slight relapse(9.4%) was found at 12 months after surgery.
The clearance rate of preoperative neck and shoulder pain were 91.2% and 90.9% at 1
month after surgery and all of these groups had no pain at 12 months after surgery.
Conclusions : Referred symptoms of TMJ internal derangement such as headache,
neck pain, shoulder pain were dramatically improved after surgical treatment of TMJ.
